Community Care Team

Newman Regional Health is a partner in a pilot program that touches the lives of Kansas in various underserved communities. Our group of frontline public health workers have an unusually close understanding of the community and the availability of resources for our residents.

The goal of the Community Care Team is to be a link between residents and the various community services that are available to them. Our team of Community Healthcare Workers interacts with people on a one-on-one basis and in small groups to educate individuals about health knowledge, self-sufficiency, social support, and healthcare services.

CHWs act as a liaison between the community member and the service providers, helping to break down language, education, or social barriers. By working with people, our team can assist with:

  • Navigating local housing, utility, or food resources
  • Enrolling in assistance programs/insurance/state assistance
  • Scheduling transportation services
  • Scheduling/attending primary care and behavioral health visits

Our community programs have a tremendous impact on our community:

HHA – Heath Heart Ambassador Program

This is a 4-month program to encourage self-monitoring and recording blood pressure as well as a heart-healthy diet.

CHWs are trained lifestyle coaches and advocates that help participants monitor and understand their blood pressure readings and medications. The team member communicates with each participant regularly on a face-to-face basis or by telephone, text, or email.

DPP – Diabetic Prevention Program

This is a twelve-month program for individuals at risk of developing type 2 diabetes. This national diabetes prevention program aims to prevent or delay type 2 diabetes, by teaching lasting lifestyle changes like eating healthier, adding physical activity into a daily routine, and improving coping skills.
