COVID-19 tests are being conducted only with a medical provider’s order. Community members MUST “call before they go” to their healthcare provider’s office if they are experiencing symptoms, have traveled recently, or have been exposed to someone with confirmed COVID-19.
Request an appointment
You can make an online appointment request with any of the providers who are part of the Newman Regional Health group. To make an appointment with providers in another clinic, you may call their office. For contact information and to search for a provider, visit our Find a Provider page.
You may request an appointment with our medical partners in cardiology, family medicine, orthopedics, pediatrics or surgery online by completing the form below. One of our appointment schedulers will respond to your request by the next business day.
Be sure to bring any diagnostic films or other test results that may have been ordered or performed by another physician.
Once you have made your initial appointment, you may pre-register by downloading, printing, and completing the forms below. Please be sure to bring them with you to your first visit. Completing the forms prior to your visit will help speed up the new patient registration process.
Please sign in to our patient portal to request an appointment and update forms. If you have not registered for our secure portal or have not received authorization to begin using the portal, call our office to obtain a registration PIN.
